Job Details

In this role, you will be responsible for providing patient evaluation, prescribing, and medication monitoring services for patients.

• Provides complete physical examinations, diagnosis, and treatment of substance use disorders.
• Administers patient-centered, quality care.
• Educates patients and/or families about preventive care, medical issues, and the use of prescribed medical treatments and/or medications.
• Orders, interprets and evaluates diagnostic tests to identify and assess patient’s clinical problems and health care needs.
• Completes patient plan health care plan summary with treatment plans and goals.
• Meets the quality and safety regulatory requirements for federal, state, accrediting agencies, health plans, and other funding sources.
• Prescribes or recommends medications or other forms of treatment such as physical therapy, inhalation therapy, or related therapeutic procedure

Qualifications

• State license to practice as a Medical Doctor in the state of AZ with full prescribing authority.
• Completion of Addiction Medicine Fellowship Program
• Board Certified in Family Medicine and Board Eligible in Addiction Medicine
• Ability to provide culturally sensitive services.
• Must have or be able to obtain a valid Fingerprint Clearance Card.
• Must possess reliable transportation, a valid driver's license, and proof of auto insurance.
• Must maintain current BLS certification.
• TB Screening Record and PPD/QuantiFERON/Chest x-ray as applicable upon hire
• Vaccinations/Titer’s Test/Declination Form: Covid, Flu, Hep B, MMR, Tdap, Varicella Suboxone Waiver upon hire date
