Job Description

The Hospice Clinical Liaison is responsible for modeling the Compassus values of Compassion, Integrity, Excellence, Teamwork, and Innovation and for promoting the Compassus philosophy, using the 6 Pillars of Success as the foundation. S/he is responsible for upholding the Code of Ethical Conduct and for promoting positive working relationships within the company, among all departments, and all external stakeholders. The Hospice Clinical Liaison is responsible for assisting referral sources in identifying patients with care needs that match the services provided by hospice.Position Specific Responsibilities include attending and participating in staff meetings, educational programs, and community events; communicating identified needs and potential solutions to the Supervisor; explaining hospice services and benefits to patients, family members, and significant others; obtaining signatures on all required forms and providing copies to patient/responsible party; ensuring all forms and medical records are completed and turned in to the Admission Coordinator; establishing and maintaining significant professional relationships; working closely with community, hospital, and facility education representatives to meet growth and development targets; facilitating the admission process to and from hospital and long-term care-based referral sources; assessing referred patient’s clinical needs and disease status and conferring with physicians regarding eligibility; facilitating patient discharge from hospital to home by coordinating transportation, DME, medication needs, supplies, and admission by a home hospice nurse; overseeing GIP patient admission process based on symptom management needs; working closely with facilities and hospitals to identify, transition, and admit patients appropriately for general inpatient services; providing education to hospitals and facilities on general inpatient care; and performing other duties as assigned.Education and Experience required include an RN License, Bachelor's degree in Nursing highly preferred, two to three years of related experience, hospital and/or long-term care clinical experience highly preferred, and healthcare marketing experience preferred.Skills required include mathematical skills, language skills for reading, analyzing, interpreting, writing, and presenting information effectively, and other abilities such as understanding, reading, writing, and speaking English and embracing hospice philosophy.Certifications require an active and unencumbered Registered Nurse license in state(s) of employment.Physical demands include standing, walking, manual dexterity, lifting up to 50 pounds, visual acuity, exposure to bodily fluids and infectious diseases, and routine use of standard medical equipment.
