Sepsis Discharge & Follow-Up Nurse Educator (Remote After Training)

Location: Fontana, CA (On-site training, then remote)
Schedule: 8:00 AM – 6:00 PM
Job Type: Full-Time

Job Description:

We are seeking a dedicated and experienced Nurse Educator with a background in inpatient sepsis care, ER, or critical care to join our growing team. This role focuses on providing patient education, discharge support, and follow-up care for individuals diagnosed with sepsis .

This position begins with on-site training at our Fontana call center, followed by remote work after a few weeks.

Key Responsibilities:

• Provide post-discharge education and support to patients diagnosed with sepsis
• Conduct patient outreach calls to review care plans, medications, and follow-up appointments
• Use EPIC to access and update patient records
• Collaborate with care teams to ensure continuity of care
• Educate patients and families on signs of sepsis, infection prevention, and recovery expectations
• Document all interactions and interventions accurately within the EMR
• Participate in ongoing training and team huddles

Qualifications:

• RN or clinical background with ER, critical care, or inpatient sepsis experience
• Strong knowledge of sepsis protocols and patient education strategies
• Proficient in EPIC electronic health record system
• Excellent communication and patient engagement skills
• Comfortable working in a call center and remote environment
• Must be available to work 8:00 AM – 6:00 PM shifts , including alternating weekends
• Able to attend initial on-site training at 17284 Slover Ave., Fontana, CA

Preferred Experience:

• Prior work in patient discharge planning or follow-up programs
• Remote care or telehealth experience
• Bilingual skills (preferred but not required)
