Job Description

The Gardens of St. Francis has an exciting opportunity for an MDS Nurse to join our leadership team that is dedicated to fostering a culture of compassion, inclusion, integrity, excellence, and collaboration in alignment with our Mission, Vision, and Values. The MDS Nurse ensures the accuracy of our resident assessments and care plan optimization. To achieve this goal, this position’s responsibilities include, but are not limited to:

  • Conducting and coordinating the development and completion of resident assessments (MDS) by current rules, regulations, and guidelines
  • Ensuring accurate and timely completion of the MDS process
  • Collaborating with the interdisciplinary team to develop individualized resident care plans
  • Monitoring and documenting resident progress and responses to care plans
  • Participating in the Quality Assurance and Performance Improvement (QAPI) process
  • Communicating with residents, families, and other healthcare providers to coordinate optimal care delivery
  • Staying updated on MDS regulations and guidelines to ensure compliance

Pay range: Up to $84,000 annually Requirements:
  • Current RN (Registered Nurse) license
  • RAC-CT (Resident Assessment Coordinator-Certified) certification is required
  • Strong understanding of MDS 3.0, RAI process, and care planning
  • Excellent organizational, communication, and interpersonal skills
  • Ability to work independently and collaboratively within a team
  • Commitment to providing high-quality care to residents
