BAYADA Home Health Care is seeking a compassionate and dedicated Registered Nurse (RN) to join our team and deliver one-on-one visits, assessments, and admissions to adult and geriatric patients in their homes. 

This is a full-time or part-time visit position offering a flexible weekday schedule and a local territory in Somerville, Malden, Revere , and nearby towns.

What You’ll Do:

• Provide skilled nursing visits in the home including wound care, medication administration, and chronic condition management
• Conduct initial and ongoing assessments to create and update individualized care plans
• Perform SOC/Admissions for new patients
• Educate patients and families to promote independence and positive health outcomes
• Collaborate with an interdisciplinary care team to ensure high-quality care

What You’ll Need:

• Active RN license in Massachusetts
• Strong clinical judgment, communication skills, and a compassionate approach to care
